2018 Member Handbook
If you are currently enrolled as a member of Banner – University Family Care, you will receive the 2018 Member Handbook by mail. We created this Member Handbook to help you access your health care plan. We also include information about your benefits, coverage, and community services. You may access the electronic version of the Member Handbook beginning October 1, 2017. Or you may request a hard copy by contacting our Customer Care Center at (833) 318-4146.
The drug formulary contains information about the drugs we cover in this planA drug formulary is a list of covered medications selected by Banner– University Family Care in consultation with a team of health care providers. The drug formulary represents the prescription therapies believed to be a necessary part of a quality treatment program.
Our plan will usually cover the drugs listed in our formulary as long as the drug is medically necessary. The prescription is filled at a Banner – University Family Care network pharmacy, and other plan rules are followed.
