The Importance of Well-Child Visits
It is important for your child to get regular well-child checkups. These visits are also called Early and Periodic Screening, Diagnostic and Treatment (EPSDT) visits or well exams. They are complete health checks for members under 21 years old.
Remember, EPSDT/well-child visits help keep your child well. Seeing the doctor when healthy can help stop problems and illness later.

This incentive offers a $25 gift card to Banner – University Family Care members ages 3–19 years, who attend their first EPSDT / Well-Child visit of 2023, between June 5 and September 5.
Any Banner – University Family Care member age 3–19 years, who attends their first EPSDT / Well-Child visit of 2023, between June 5 and September 5.
Please allow 4–6 weeks after the date of the EPSDT / Well-Child visit, for delivery of the gift card.
This card will not allow the purchase of products such as tobacco, alcohol or firearms.
No. Each household member age 3-19 years, who attends their first EPSDT / Well-Child visit of 2023, during the campaign will be eligible for the gift card.
