Due to the ongoing COVID-19 pandemic, it is important that everyone 6 months and older gets a flu shot this year.
Roll up your sleeve and get a flu shot today. Call your PCP to schedule an appointment. For help in making an appointment or to arrange transportation, call our Customer Care Center at (800) 582-8686, TTY users call 711.
Members who get their flu shot qualify to earn a $10 Subway gift card! Gift card will be mailed to your address on file about 6 weeks following your flu shot. The gift card will be distributed under the Banner Health Plan when dually eligible.
