Members can find Banner – University Family Care/AHCCCS Complete Care (BUFC/ACC) plan information below. Plan materials are effective on October 1, 2018. Members should receive their new 2019 plan information packet in the mail by October 1, 2018. If you are a BUFC/ACC member and have not received your 2019 plan materials, please call our Customer Care Center. 

Member Handbook

If you are currently enrolled as a member of BUFC/ACC, you should have received the Member Handbook. We created this Member Handbook to help you access your health care plan. We also include information about your benefits, coverage, and community services. If you need more information you can call our Customer Care Center.

Drug Formulary

The drug formulary contains information about the drugs we cover in this plan

A drug formulary is a list of covered medications selected by BUFC/ACC in consultation with a team of health care providers. The drug formulary represents the prescription therapies believed to be a necessary part of a quality treatment program.

Our plan will usually cover the drugs listed in our formulary as long as the drug is medically necessary. The prescription is filled at a BUFC/ACC network pharmacy, and other plan rules are followed.
