Members can find Banner – University Family Care/ACC (B – UFC/ACC) plan information below. Plan materials are effective as of October 1, 2022, through September 30, 2023.

Members, please review your 2022/2023 plan information. If you are a B – UFC/ACC member and have not received your 2022/2023 plan materials in the mail, please call our Customer Care Center. 

Member Handbook

If you are currently enrolled as a member of B – UFC/ACC, you should have received your Member Handbook. We created this Member Handbook to help you access your health care plan. We also include information about your benefits, coverage, and community services. If you need more information, you can call our Customer Care Center at (800) 582-8686, TTY 711.

Drug Formulary

The drug list contains information about the drugs or medications we cover in this plan.

A drug formulary is a list of covered medications selected by B – UFC/ACC in consultation with a team of health care providers. The drug formulary represents the prescription therapies believed to be a necessary part of a quality treatment program.

Our plan will usually cover the drugs listed in our formulary as long as the drug is medically necessary. The prescription is filled at a B – UFC/ACC network pharmacy, and other plan rules are followed.
